Before Your First Visit:

Before scheduling a visit, you can call for a FREE 15 minute consultation that will help you to find out how our services may be able to help you with your health problems. You can ask any questions and then decide whether or not you would like to schedule your first office visit. The free consultation is not intended for medical advice, to give you a medical diagnosis, or to recommend a specific treatment program.

When you schedule your appointment the staff will direct you how to access the online intake forms which are required to be filled out before your first appointment. If you do not have access to email or computer, you can fill out the following paper intake forms.
It is important that you complete these forms before your appointment so that we can make a full assessment of your health. Please give yourself one hour to thoroughly fill out intake forms. Forms may be opened in your browser or downloaded directly by “right clicking” over document and selecting “Save Target As” or “Save Link As”. Forms are provided as PDF which can be viewed in Adobe Acrobat Reader.

During Your First Visit
Your first visit with the doctor will set the foundation for your health plan. The initial visit includes a comprehensive evaluation of your health issues and your relevant medical history, followed by a physical exam if needed, and a recommendation for treatment, diagnostic testing and/or herbal medicine or nutritional supplements. Please bring any recent laboratory results that you may have and any nutritional supplements you are currently using to your first appointment. The initial visit usually lasts about 1.5 hours.
If you are unable to keep your appointment for any reason, please reschedule as soon as possible. This will allow us to offer that time to another patient who may be waiting to get in.
We ask for your credit card number to reserve your initial appointment time. If you miss your appointment or cancel within 48 hours, we will charge your card $100. Payment is required at the time of service. We accept cash, Master Card, VISA, American Express and Discovery. We look forward to meeting you and providing you with the best natural health care available.
Insurance:
While insurance companies are not required to cover Naturopathic Doctor services in California, many health insurance are starting to. Ask your insurance company whether or not they cover Naturopathic Doctor services. If your insurance does, you can request a “Superbill” from us that you can submit it to your insurance company for possible reimbursement.
Aside from your monthly health insurance premium, if you constantly pay a lot of additional out-of-pocket, co-pay expenses for routine medical appointments, prescription drugs, blood and other laboratory tests, etc, Functional Medicine may be a good alternative. Your ND works with you to design a health plan that is focused on long term healthy living, not just reducing your symptoms in the short term.
If your insurance that does not cover these services, a Health Savings Account (HSA) or flexible savings plan may be able to cover your care. Speak with your employer to see if any options are available. If you do not have any naturopathic coverage, let your insurance company know that this is something you would like provided in the future.
